The following forms should be downloaded, completed, and returned to the Admissions Office along with your $100 application fee. You may also request a hard copy of the application packet in the mail by contacting us.
A complete application includes:
- Application for Admission
- Kindergarten Teacher Recommendation Form (Completed and returned by teacher)
- First through Fifth Teacher Recommendation Form (Completed and returned by teacher)
- Permission for Information Release Form (Given to current school’s office to obtain official transcripts)
- Gesell Assessment (Grades K and 1 only)
- Student Day visit
- Parent Campus visit
- $100 non-refundable application fee
